Post Acute Transitional Care
Post Acute Transitional Care which is defined as a level of care needed by a patient who does not require acute inpatient hospital care, but who requires more intensive skilled nursing care, higher than that provided in a long term care setting. Transitional Care is provided to those patients who require short term rehabilitation and specialized care in order to make the transition from hospital to home. A program of intense treatments of therapy, it is also a comforting choice to those who are either too ill or those who are dependent on in-home support services. At Sherman Oaks Health & Rehabilitation Center, our team of professionals will provide physician directed care, while creating a comprehensive care plan designed to enhance your rapid return to home.
